If you sign up to have your credit card billed for 12 months, please note that you will be charged on the next billing day of this month and the remaining 11 months. For example, if you complete this form on August 22nd, you will be charged on August 23rd, September 23rd, October 23rd, etc. You will receive a receipt for each charge.
